PT/PTA | Criminal History Background Check
Pursuant to W. Va. Code §30-20-8a and W. Va. Code §30-20A-4.6, applicants seeking initial licensure by the West Virginia Board of Physical Therapy are required to request and submit to the Board the results of a fingerprint-based state and national/federal criminal history record check. Please be aware that criminal history record checks may take several weeks to process and cannot be expedited for any reason. Applicants should not request and submit to the Board the results of a criminal history record check until after they have completed a licensure application and paid the appropriate licensure fees. Challenge of Record Information
Fingerprinting services are provided by IdentoGO for a fee of $45.75. Results will be sent electronically to the West Virginia Board of Physical Therapy from the West Virginia State Police and the Federal Bureau of Investigation. By law, the Board cannot use fingerprint results that were not sent directly to the Board. In the event that your fingerprints are rejected, notification of the rejection will be sent from the Board directly to you via email. If your fingerprints are rejected, you must be fingerprinted again.
