The WV Board of Physical Therapy has the responsibility of investigating any wrong-doings of licensed Physical Therapists and Physical Therapy Assistants. We take any allegation of illegal or immoral behavior very seriously and investigate any complaints as quickly and thoroughly as possible. In the event that a PT/PTA has been found to have violated any of the Rules or Regulations pertaining to their profession, the Board will issue a penalty appropriate for the infraction. All disciplinary actions issued by the Board are a matter of public record.
THE LISTING BELOW MAY NOT BE A COMPLETE LIST OF ALL PAST OR CURRENT DISCIPLINARY ACTIONS. CASE FILES THAT ARE OPEN AND BEING INVESTIGATED DO NOT GET LISTED UNTIL THE CASE HAS BEEN RESOLVED OR CLOSED.

A current verification of an individual PT's or PTA's License may be obtained directly from the Board Office by making a written request and submitting the required fee in the amount of $25.00. Please submit the fee in the form of a Money Order or Business check along with your request to the Office address.
A LISTING OF ACTIVELY LICENSED PT'S/PTA'S CANNOT BE USED FOR VERIFICATION PURPOSES NOT CAN IT BE COPIED OR DISTRIBUTED. WE DO NOT RECOMMEND THAT INTERESTED PARTIES ASSUME THAT ALL ACTIVE LICENSEES ON A LISTING ARE IN GOOD STANDING. WE DO RECOMMEND THAT REQUESTS BE MADE FOR INDIVIDUAL VERIFICATIONS AS PER THE PARAGRAPH ABOVE.
If you believe a PT/PTA has violated any Rules or Laws pertaining to the practice of their profession, you may formally submit this for review to the Board by accessing a Complaint Form from the "Complaint Info" page under the "PT/PTA" section of this website.
PERSONS MAKING COMPLAINTS ARE IMMUNE FROM LIABILITY FOR THE ALLEGATIONS CONTAINED IN THEIR COMPLAINTS FILED WITH THE BOARD UNLESS THE COMPLAINT IS FILED IN BAD FAITH OR FOR A MALICIOUS PURPOSE ACCORDING TO TITLE 16, PROCEDURAL RULE, SERIES 3 "DISCIPLINARY AND COMPLAINT PROCEDURES" 16-3-5.3.
You would then complete the form and mail it into the Board Office. The form must be signed and dated. Once the Complaint Form is submitted, it will become public information and may be disseminated as necessasry in order to investigate and decide the alleged
incident. We cannot guarantee or promise that the information you submit will be kept confidential or that you will be able to remain anonymous.
ACCORDING TO SECTION 16-3-5.3 "COMPLAINANTS ARE IMMUNE FROM LIABILITY FOR THE ALLEGATIONS CONTAINED IN THEIR COMPLAINTS FILED WITH THE BOARD UNLESS THE COMPLAINT IS FILED IN BAD FAITH OR FOR A MALICIOUS PURPOSE."
