THESE INSTRUCTIONS WILL BE UPDATED TO INCLUDE INFORMATION REGARDING THE AVAILABILITY OF RENEWING ONLINE.

Licenses for the upcoming period of time will be run and mailed out in the order we receive them, beginning in October 2009.

INCOMPLETE FORMS or INCORRECT FEE AMOUNTS: This will result in your License Renewal being delayed--- as any incomplete forms or incorrect fee amounts will be returned to you. Be sure to mark in the designated box which is your preferred mailing address - - either your residence (front of the Form), or your employer location (back of the Form).

If your Facility or Company is sending in your Renewal Certificate(and remitting your fee), you need to indicate this on the Renewal Certificate:
(1)If the "License Only" is to be mailed to your Facility, and all other mail sent to your residence, OR
(2) If the "License" and all other mail is to go to your residence address - you need to
check the box on the front of the Renewal Certificate for "Home preferred address", OR
(3) If ALL mail is to go to the business or
Facility, check the box "for Employer preferred address" on the back of the
Renewal Certificate.

In order for us to receive, process and return your new License to you which would be effective January 1, 2010, you must allow the Board Office an adequate amount of time to update your information and issue your new License. Renewals postmarked 12/31/2009 cannot be processed in time for you to work beginning January 1, 2010. Please keep this in mind. You cannot work without a current License.

It is the recommodation of the Office Staff that you attempt to have your renewal PROCESSED and fee PAID by December 15, 2009 in order to allow us enough time to process it and get it back out in the mail to you.

AGAIN, THESE INSTRUCTIONS WILL BE UPDATED WITH INFORMATION PERTAINING TO RENEWING YOUR LICENSE ONLINE.
