PT/PTA | Complaint Info & Complaint Form

The Filing of a Complaint against a Physical Therapist or Physical Therapist Assistant is a matter of serious consideration.

If in the course of you being treated by a Physical Therapist or a Physical Therapist Assistant, you believe that something was said or done that was inappropriate, you may file a Complaint. You must be willing to document the date(s) and circumstances surrounding the incident. You must be willing to list the names and contact information of any witnesses to the incident. Lastly, you must be willing to sign and date the form realizing that it will now become a public record..

Complete this form to submit a complaint regarding a PT/PTA to the Board - mail to: 101 Dee Drive, Charleston, WV 25311

If it is determined that a Complaint needs to be sent to the Board, the process that will ensue will involve copying the Complaint and any accompanying documentation to the Disciplinary Committee Members as well as the Respondent in order that the allegations may be responded to. The Disciplinary Committee will gather all information they feel would be helpful in their determination of a recommendation to the Board of "No Probable Cause" or "Probable Cause". If necessary, an Ethics Investigator can be hired to interview the parties involved and to also collect any pertinent documents. According to Law, the Complainant is due an update in 6 months after the Complaint is initially filed, if it is not resolved by in that time frame. The Case must be fully resolved after a total period of 18 months from the date of initially filing unless the parties involved agree to an extension.
