Online License Search / Verifications Request
PT/PTA | Online License Search / Verifications Request
If you are a licensed PT/PTA requesting a verification to be sent to another jurisdiction, please refer to "PT/PTA | Licensed PT/PTA-Letter of Good Standing".
DO NOT USE THIS FEATURE FOR PERSONS WORKING ON TEMPORARY PERMITS.
1. Scroll to the bottom of the page and click on "License Search"
2. Type the last name and license number
3. Click “Search License”
4. Click on the name or the licensee
5. Click on “Checkout-Purchase Documents” Payment to be made by credit card
or by check. If you choose to pay by check, please make sure you have a pre-paid account set us with the office or promptly mail a business check so that we can release the verification you purchased to your email address.
6. Follow credit card instructions to complete your purchase.
A receipt will be sent to your email address from the Treasurer’s Office. You will also receive the verification/s that you purchased in an email that you will need to open and download.

The above link will only give you the PT/PTA’s license number and will not show the licensure status of the licensee. However, a true verification of licensure can be purchased here for $25 that gives the licensees status & disciplinary actions.
