INSTRUCTIONS TO RENEW ONLINE:

If you are mailing in your renewal, we only accept money order or chashier's check. We do not accept personal checks.

User Name: First letter of first name
w/last name (e.g. EBaker)
Password: Last 4 digits of SS#
User Name and Password may be entered at the following location:
http://www.wvlicense.com/sites/wvbopt/
Log In, then click the "License" link at the top, select "Original" license, and then click the "option to renew" (lower left hand corner). From here it will guide you through the rest of the process.
NOTE: If changes to your permanent information as it appears needs to be made, it must be done prior to you renewing by forwarding the updated information to the Office via fax, or email. You will have to sign and attest that the information shown online is correct prior to paying with a credit card. There is a $3.00 convenience fee that is added to the required total.
If you are taking Inactive Status, please fax front & back of Part C/D of your Form to the Office address; or mail it...or email any changes to your permanent information. Information changes cannot be made online. The $25.00 fee required can be paid online or mailed to the Office. Please make payment to the "WV Board of Physical Therapy".
All Licenses not renewed by midnight 12/31/2009, will automatically be charged the late fee in addition to the License Fee.
