||It was brought to the Board's attention that Logan's Ohio license was revoked on or about July 22, 2021. Logan failed to report this disciplinary action against her Ohio license in violation of W. Va. Code R. §16-1.8.8 requiring a licensee to report such discipline to the Board within 30 days of that discipline. During the Board's investigation of this matter, it was revealed that Logan failed to complete the necessary 24 hours of Board-approved continuing education within the applicable two-year (2019-2020) licensing period in violation of W. Va. Code R. §16-1-11.2. Documentation submitted by Logan revealed that she fell 3.75 hours short of the 24 hours needed to remain active and in good standing.
||Logan shall, at her own expense, make up 3.75 hours of Board-approved continuing education for the 2019-2020 reporting period. Logan shall submit written proof of completion of these hours to the Board. These makeup hours are in addition to what is otherwise required in order to maintain licensure as a physical therapist assistant during the current reporting period and these make-up hours shall be completed within 60 days of the entry of the consent agreement and order.
Logan shall also, at her own expense, enroll in and successfully complete six hours of Board-approved course(s) in ethics. This shall be completed within 60 days of the Order.
Logan shall be subject to a mandatory audit of her continuing education hours prior to her next renewal of her license.
Logan shall pay a continuing education non-compliance fee in the amount of $200 within 60 days of entry of the Order.
