Violated Law: W. Va. Code §30-20-1, W. Va. Code §30-20-14
Violation: Tammy Stanley, PTA failed to renew her license by December 31, 2017 as required by applicable statute and regulation. On January 1, 2018 the Board placed Ms. Stanley on delinquent status and her employer was notified. By letter dated January 12, 2018 Ms. Stanley acknowledged that she had not renewed her license and that she continued to work as a physical therapy assistant. Ms. Stanley submitted an application to reactivate her license on January 12, 2018. The board reactivated her license effective January 18, 2018.
Penalty: Ms. Stanley shall complete six (6) hours of continuing education in the area of ethics which have been board approved within 60 days from entry of order. These six (6) hours are in addition to the mandated hours to maintain licensure as a physical therapy assistant and may be obtained online.
