PT/PTA Name: Tasha Hanks, PTA
Violated Law: W. Va. Code R. ยง 16-1-10.2
Violation: Tasha Hanks failed to renew her license by December 31, 2017, as required by applicable statute and regulation. On or about January 2, 2018 Ms. Hanks brought to the board office her reactivation application and 2015 continuing education certificates. Ms. Hanks did not submit any continuing education certificates for the 2016-2017 reporting period. She acknowledged to the Board's Executive Director that she did not complete any continuing education for the 2016-2017 renewal period. Ms. Hanks subsequently completed the necessary continuing education to reactivate her license.
Penalty: Ms. Hanks shall complete six (6) hours of continuing education in the area of ethics which have been board approved within 60 days from entry of order. These six (6) hours are in addition to the mandated hours to maintain licensure as a physical therapy assistant and may be obtained online. Ms. Hanks shall also be subject to a mandatory audit of her continuing education hours prior to her next renewal of license. She shall contact the Board prior to submitting a renewal application so that the Board can confirm she has the necessary continuing education hours for renewal.
